This research aim to Development of Tele-nursing Model of Chronic Disease Elders in Sansai district, Chiang-Mai Province. This research was Research and Development; R&D This aim to Development of Tele-nursing Model of Chronic Disease Elders and used the concept of Tele-nursing with the nursing process in developing. This study composed of 2 phases. phase 1 study the concept of Tele-nursing, using the principles and exploring the problems and needs of the elderly with chronic diseases through the Tele-nursing questionnaire and in-depth interviews with executives and professional nurses who provide consultation via tele-nursing system. To explore the possibility of arranging a tele-nursing model to suit the context of the actual situation. phase 2 to study the effectiveness of the Tele-nursing Model in the elderly with chronic diseases on the quality of life of the elderly using quasi-experimental designed. (Phase 2 requesting research budget from the Association of Nurses on the Elderly Therefore, the results have not been summarized in this research report). The population is 32,880 elderly people with chronic diseases in San Sai District. The sample size total 397 people by simple random sampling. The study was conducted in 12 villages in Mae Faek Subdistrict Municipality, San Sai District, Chiang Mai Province. The samples were 10 professional nurses and 3 hospital directors who were Purposive sampling. The study was conducted between December 2022 and May 2023. Results: the first phase of the study, the researcher studied situation of problems in using the service and the need for using Tele-nursing Model as follows: 1) Travel Problems to Sub-District Health Promoting Hospital, There were very problems (Mean = 3.50, SD = 1.29) 2) The problems of follow-up care from nurses, There were moderate problems (Mean = 2.56, SD = 1.03) 3) The problems gaining knowledge about chronic disease care from nurses, There were moderate problems (Mean = 2.56, SD = 1.02) 4) Problems with travel expenses, There were moderate problems (Mean = 2.87, SD = 1.26). 5) Problems in communication channels with nurses, There were moderate problems (Mean = 2.69, SD = 1.12) 397 elderly people with chronic diseases were interviewed did not know the meaning of Tele- nursing, 86.1%. and some know 13.9 % but when asked to explain the meaning of the sample, still unable to explain correctly. The sample group thought that using smartphones to communicate with nurses was difficult. accounted for 45.8 % and thought it was not difficult 54.2 % and semi-structured in-depth interviews with 10 registered nurses and 3 executives involved in policy-making. it was found that 1) the provision of services through the form of Tele-nursing are suitable for the current situation But there should be a pattern. or make a policy for nurses and should have the workload of online counseling as well Emphasis is placed on the nursing process according to the nursing council. 2) Confidentiality During tele-nursing, the confidentiality of the elderly with chronic illnesses must be maintained. in accordance with the Personal Data Protection Act B.E. 2562 or related laws. 3) budget, a clear policy should be established starting from the host hospital system. down to the client. Therefore, the model of Tele-nursing in the elderly with chronic disease in San Sai District Chiang Mai Province consists of 1) Tele-nursing system for the care of the elderly with chronic illnesses with Sansai Hospital as a host 2) Line application in providing nursing care for the elderly with chronic diseases according to the 5-step nursing process 3) Handbook of nursing practice through a remote nursing model in the elderly with congenital disease. 4)A training program to enhance skills for the elderly with chronic diseases in using the remote nursing model.
